lifting the adoption registration：1.Adoption registration authority has jurisdiction The adopter and the person having placed out the child or the adopter and adult adoptee agree to terminate the adoptive relationship；If the adopted child involved reaches the age of 10 or more, his or her consent shall be obtained. Application materials: cannot be ascertained and they have been raised in the city social welfare organizations:1.Adoption registration application;2.Documents and Certificates:(1) Documents and Certificates by the Adopter(s):① Adoption Application: reason(s) and aim(s) for adoption and a pledge of not abandoning or abusing the adoptee but raising the adoptee to grow up healthily; ② Passport and Resident Certificate; Hong Kong Identity Card and Pass for Multi-entry into Mainland Held by Resident of Hong Kong; Macao SAR Resident Identity Card and Pass for Multi-entry into Mainland Held by Resident of Macao; Taiwan Identity Card and Pass for Multi-entry into Mainland Held by Resident of Taiwan; valid tourist certificate issued or endorsed by the organs-in-charge of People’s Republic of China; ③The adopter shall provide papers certifying such particulars of the adopter as age, marital status, whether the adopter has a child,profession, property,health and whether ever subjected to criminal punishment,which are issued by the competent agencies of the country to which the adopter belongs.Such certifying papers shall be authenticated by a foreign affairs institution of the country to which the adopter belongs or by an agency authorized by the said institution,and by the embassy or consulate of the People’s Republic of China stationed in that country concerned (or issued by the notary public in HK entrusted by the organ-in-charge of the state,by competent agencies of Macao,or by Public Notary Office in Taiwan Region).（2）Documents and Certificates by institutions placing the adoptee for adoption：①responsible person ID card (copy). ② abandoned babies,child welfare agencies access to original records and the collective household registration documents. ③ issued by public security organs to pick up abandoned children, children report ,and orphans whose parents died or declared dead the court evidence.④ find an abandoned baby, children’s biological parents notice (above the county newspaper at least 60 days). ⑤ the adoptee is a child with disabilities, should be submitted to the county level or above issued by medical institutions to prove the child's disability.⑥ adoption agreement and other relevant information.⑦Letter of Intent: the adoptee aged above ten must write a letter of intent to be adopted.3.Photos:two 2-inch recent high-length Bareheaded group photos of the adopter(s) and adoptee. The consent Adopted written submissions issued by others who has obligations to foster ; ⑤ proof of taking actual custody . Units from the orphan's parents alive or orphan village (residence) committee that issued the custody of the responsibility.（3）Documents and Certificates by the Adoptee：①registered household booklet; ②Letter of Intent: the adoptee aged above ten must write a letter of intent to be adopted。3.Photos: two 2-inch group photos of the adopter(s) and adoptee，two 2-inch recent Half-length Bareheaded group of single photos of the person who place out the children for adoption. Lifting of adoption registration: 1.Lift the adoption registration; 2. Termination of adoptive relationship agreement; 3.Adoption registration certificate issued by adoption registration authority ; by a notary public to establish adoptive relationship shall obtain a certificate; 4. Adopters、adoptee and the person who place out the children for adoption must take their ID card, residence booklet. / / Adopted people are social welfare organizations, social welfare agencies to submit the copy of the legal representative of residents of identity card. 5.Two 2 inch recent high-length Bareheaded single photos of Adopters、adoptee and the person who place out the children for adoption,except for social welfare agencies Adopted.
